The situation was truly dire. I hadn’t slept a wink last night, working until dawn, and today, we had a patient in the surgical department weighing over 140 kilograms. He couldn’t even lie flat because the weight of his body would compress his chest cavity, making it difficult for him to breathe. However, he urgently needed a central venous catheter (CVC) inserted—his wound was severely infected and required intravenous antibiotics, but we couldn’t find a vein in his arms to administer the medication. I prayed that I wouldn’t have to perform the procedure, but the attending physician assigned it to me. There was no choice but to face it head-on. I walked into the room. The patient looked very frightened and said he didn’t want to lie there for another minute. However, he also indicated that he understood the situation and was willing to cooperate as much as possible. We discussed it and decided that he would sit up on the bed for the procedure until he absolutely had to lie down. I thought, let’s take it one step at a time. I prepared everything: reviewed the medical history, arranged the instruments, and placed the rolled towels properly, among other tasks. He leaned back against the bed while I wiped his chest with a disinfectant swab. This time, Dr. Su was by my side to guide me. Once everything was ready, I asked Dr. Su to help him lie down and put an oxygen mask on his face. The fat on his chest rose and fell like waves, and my fingers couldn’t even feel his collarbone. By this point, he was breathing rapidly and turning red in the face. I gave Dr. Su a questioning look: “Do you want to take over?” She gestured for me to continue, so I had no choice but to inject him with anesthetic, bracing myself as I estimated where to insert the needle and then aimed that thick needle at that spot. I focused intently on stabilizing the needle and slowly withdrew the syringe while threading the guidewire through smoothly. At this point, his breathing became very labored again. We had to let him sit up for a moment; this eased his breathing slightly. Then we asked him to lie back down again while I quickly attached the dilator and positioned the catheter. Dr. Su said “Well done” before leaving. I still couldn’t figure out what had been different about that day compared to my previous attempts. However, from that point on, I was able to successfully place catheters for patients without issue. I suppose this is what practice can achieve. By now, I have placed over 100 central venous catheters, but that doesn’t mean it’s foolproof; once, I punctured a patient’s right lung. Of course, there have been several instances where everything went perfectly—no hesitation or second-guessing; just holding the needle and inserting it into the patient’s chest while feeling the pathway of the needle—first through fat, then firm muscle, followed by entering the vein itself.
