My second experience with central venous catheter (CVC) placement was no better than the first. This time, the patient was seriously ill and in the intensive care unit, and we needed to implant a CVC to deliver medication directly to her heart. Despite perfect initial preparations, the actual procedure was a disaster. The needle was either too shallow or too deep, and repeated failures left me hesitant and unsure of what to do next. I kept trying but made no progress. Suddenly, I noticed blood in the syringe, which indicated that the needle might have entered the vein. I stabilized the needle with one hand while trying to remove the syringe with the other, but it was stuck tight and wouldn’t budge. I applied more force, but this caused the needle to slip out of the vein, resulting in fresh blood gushing from the puncture site. I pressed down to control the bleeding, but a hematoma formed around her chest cavity. The hematoma made it impossible to insert the catheter further. I considered giving up, but the patient needed this line, and my supervising senior resident—who had been working for two years—urged me not to quit, believing I could succeed. I sent the patient for an X-ray to confirm that my previous actions hadn’t harmed her lung. The senior resident handed me a brand-new set of tools and encouraged me to try again. However, I still failed; it felt like I was turning her into a pincushion. Finally, my senior resident couldn’t watch anymore and took over. Even he took a few minutes and several attempts before locating the vein, which made me feel better; perhaps this patient's situation was particularly challenging. ### Unexpected Success Many people often think that only those with perfect hands can become surgeons, but that’s not true. When I applied for surgery, no one checked whether my hands were flawless or conducted tests to assess my suitability based on my hand’s condition.
