Andy is a small, thin elementary school student with a spiky haircut. He had always been healthy, but in recent weeks, his mother noticed that he had a persistent cough and seemed lethargic. Just two days ago, he could barely eat, and his mother suspected he might have caught a cold. However, that night, he appeared pale, shivering all over, and gasping for breath; suddenly, he was struggling to breathe and was immediately taken to the community hospital.
In the emergency room, doctors placed him on a ventilator to assist his breathing and initially suspected an asthma attack. However, when the X-ray results came back, they discovered a large mass in the center of his chest cavity. To gather more detailed information, they performed a CT scan. The black-and-white images clearly showed a mass the size of a cantaloupe that was dense and pressing against the blood vessels surrounding his heart, pushing the heart to one side and compressing the airways between his lungs. His right lung was completely collapsed, with no air entering at all. The fluid secreted by the mass filled his right chest cavity, leaving Andy reliant on his left lung for breathing, while the mass also compressed his left bronchus. The community hospital lacked the capacity to handle such a situation, so they transferred him to our hospital for further treatment. Our hospital has a strong medical team and top-notch equipment, but even that does not guarantee we can cure him.
When Andy was admitted to our intensive care unit (ICU), his breathing was accompanied by a "buzzing" sound that could be heard from two or three beds away. This indicated that he was at risk of death; simply lying down could cause the mass to obstruct his airway and suffocate him. The sedatives or anesthetics we administered could similarly put him in danger. At this point, it was impossible to surgically remove the mass. Chemotherapy is known to be an effective method for eliminating such masses but requires several days. The pressing question was how to buy this child some time; whether he could survive the night remained uncertain.
